Payment for your co-pay or deductible

To receive a copy of your reports please sign up for our Patient Portal and download them.
If you need reports and/or images on a CD, you can walk into either location and request them from the front desk Monday-Friday 8am-12pm and 1pm-3pm. Please note that there may be a wait.
